Today is the last day for me to post about FAQs or Frequently asked Questions. Actually, it ended yesterday but today, I’ll give you a recap.

So, to summarize, you can go file in or submit in your application at UP ISSI E. Virata Hall E. Jacinto St. UP Diliman, Quezon City from Monday to Friday at 8am-5pm from May 12, 2014 until November 10, 2014.

And call for the one’s whose area is close to LUCENA. HRVCB or the HUMAN RIGHTS VICTIMS CLAIMS BOARD will be there TOMORROW, June 16 and 17 at Bulwagang Kalilayan Provincial Capitol Compound Quezon Ave., Lucena City.

Don’t forget to bring in your ORIGINAL documents along with a PHOTOCOPY of your documents. Your affidavits should be NOTARIZED already for FASTER processing. And note that the board will only need ONE COPY of your AFFIDAVIT so if you are planning to have it notarized, only prepare one copy for them.

If you are the victim, you only need an application form, a duly notarized sworn statement and 2 government issued IDs. It would be up to you if you would like to submit in your supporting documents but it is HIGHLY encouraged for you to do so to give weight to your case. Okay?

Your supporting documents could also be sworn statements of 2 of your co detainees or 2 people who have knowledge of what happened to you (duly notarized, of course) if you don’t have clippings, photos, release papers, ASSO or other documents to present.

If you are the representative, aside from the aforementioned, you also need to prove your FILIATION to the victim. It’s either you provide a birth certificate of you or the victim or marriage certificate (if claiming for your husband/wife) if you can’t provide such documents, duly notarized sworn statement of 2 disinterested person who personally knew your filiation with the victim is needed.

They’ve had reports of a FIXER that gives out copies of TORTURE CLAIM from HAWAII for a promise of 10% of how much they’ll get from the board once they’ve claimed. I’m telling you now, DON’T BELIEVE FIXERS! IF THEY’RE NOT FROM HRVCB, THEY CANNOT HELP YOU!

Once more, their APPLICATION FORM is free and if the info I got is correct, they have the forms translated to Filipino and Visayan too and is free to download from their website.



1. The documents that you would submit would temporarily stay at the care of HRVCB at the time of processing and would later be permanently transferred to a memorial, museum or library. If you want to keep your original documents, you can submit the certified copy of this. You can certify it on site with the Officer of the Day.

2. Direct or Indirect commission of fraud or any other collusion, falsification, misrepresentations of facts, or any other kind of anomaly in the accomplishment of this form, or in obtaining any benefit under this application shall be subject to CRIMINAL ACTION punishable by 8-10 years imprisonment and other accessory penalties as provided for under Republic Act No. 10368, Sec. 25.
